The following documents will be REQUIRED for all players for BOY'S State Tournaments

Copy of Report Card or Grade Report Form  and Walker ID, Military ID, or Passport

Walker ID's (No BC's required)

How Do I Get a Walker ID (aka child ID card)

Parents can obtain ID cards for their children under age 15 at any DMV customer service center.

• Issued to a child under age 15.
• Requires one identity document for the child such as a birth certificate or passport.
• Parent must certify the child's identity and residency using the Identification Card Application for Minors .
• Expires on the child's 16^th birthday.
• If the child's name appears on the parent's legal presence document, that document governs the expiration date of the child's legal presence.
• Costs $10.00

• You will need original documents that prove the following:

  • Identity
  • Legal presence in the United States
  • Social security number, if you have been issued one
  • Virginia residency

• One of the following documents is required along with the child’s birth certificate.

  • Certified copy of school records/transcripts issued by a school in which the applicant is currently

  enrolled and the school must be accredited by a U.S. state, jurisdiction or territory OR a Virginia

  Department of Education Certificate of Enrollment form. (A report card is not accepted.)

  • Monthly bank statement not more than two months old issued by a bank

  • Current automobile insurance policy or life insurance bill

  • Payroll check stub issued by an employer within the last two months

  • U.S. Internal Revenue Service tax reporting W‐2 form or 1099 form not more than 18 months old.

  • U.S. or Virginia income tax return from the previous year

  • Deed, mortgage, monthly mortgage statement or residential rental/lease agreement.

• Utility bill, not more than two months old, issued to applicant. Examples include gas, electric, sewer,

  water, cable or phone bill. Cellular and pager bills are not acceptable.

  • U.S. Postal Service change of address confirmation form or postmarked U.S. mail with forwarding address label.

  • Virginia Voter Registration Card mailed to you by your local registrar

  • Receipt for personal property taxes or real estate taxes paid within the last year to the Commonwealth of Virginia or a Virginia locality

  • Cancelled check not more than two months old with both name and address imprinted

  • Annual Social Security Statement for the current or preceding calendar year.
